If your needs are more complicated, or specialist care is required, our continuing care teams can provide clinical support. This care can be provided for a range ofcare needs including long term and complex conditions such as Dementia, Parkinsons or Multiple Sclerosis, palliative and end of life care, rehabilitation from head and spinal injuries, home ventilation and respiratory support, learning or mental health disabilities or at home renal care.
Our complex care teams are coordinated by a lead nurse who meets regularly with your healthcare consultants, practitioners, you and your clinical care team to make sure the care you receive meets your preferences and choices. As well as being trained in line with all national care standards, the team we provide will also receive training specific to your clinical needs, so we can be sure you are in the best hands.
With the advancement of technology, and a real governmental push towards care in the community, we are proud that we can now help hundreds of clients with extremely complex care needs return to the comfort of their own homes. Receiving one to one care in this way better meets your physical, emotional and psychological needs by allowing you better choice in your care, far more independence and the option to stay in your own home.
